At Stryker, we know that fear is a primary reason that people put off going to the doctor, and we want to change that. Through our “Get on the Bus” contests and our athlete ambassadors, we want to help joint pain sufferers start the conversation with their doctors and actively manage the pain.

Beginning in June, Jerome Bettis and Fred Funk will hit the road with their Stryker-themed bus to greet three grand prize winners! Each winner will spend time together with Jerome, Fred, or both, and talk about the impact of joint pain on the body and why it’s important to address it with a doctor. Winners will also catch a ride to/from their doctor’s office to learn how the joint pain they are experiencing can have long-term impact on their health.

“2018 Stryker Get on the Bus Florida Contest” Official Rules (“Official Rules”)

 

NO PURCHASE NECESSARY TO ENTER, AND SUCH A PURCHASE DOES NOT IMPROVE YOUR CHANCES OF WINNING.  VOID WHERE PROHIBITED BY LAW.

 

1. HOW TO ENTER:  To enter the “2018 Stryker Get on the Bus Florida Contest” (“Contest”) visit patients.stryker.com (“Site”) during the Contest Period, accurately and truthfully complete the online registration form, accept these Contest Rules, and follow the instructions for submitting an Entry as described on the Site and below. An Entry must be an original essay of 200 words or less in the English language describing why the Entrant or Entrant’s loved one needs an extra push to visit the doctor. The Entry must be submitted through the Site.  The party accepting these Contest Rules and submitting an Entry is referred to as “Entrant” or “you.”  Proof of submitting an Entry other than as detailed in these Official Rules (such as a printed or copied confirmation screen or message) does not constitute actual receipt of the Entry for purposes of this Contest.

 

The Contest will begin April 10, 2018 at 12:01 AM Eastern Daylight Time (“EDT”) and conclude June 15, 2018 at 11:59 PM EDT (“Contest Period”).  All Entries must be submitted within the Contest Period.  The winning Entry will be selected by June 25, 2018.  Stryker Corporation (“Stryker” or “Sponsor”) is the official keeper of time for this Contest. 

 

Each Entrant represents and warrants as follows:

 

  • Each Entry does not infringe or otherwise violate any rights of any third party or entity, including without limitation Intellectual Property Rights as defined below, defamation, privacy, publicity, false light, misappropriation, confidentiality, or any contractual or other rights.
  • Your entire Entry is an original work by you, and you have all the rights, licenses, permissions, and consents necessary to submit the Entry and to grant all of the rights that you have granted hereunder.
  • Your Entry does not contain content that is inconsistent with the permissible uses outlined by these Contest Rules, including without limitation content that is unlawful, harmful, threatening, abusive, harassing, defamatory, libelous, invasive of another’s privacy, vulgar, profane, sexually explicit, obscene, racially or ethnically offensive, or otherwise objectionable as determined by the Sponsor in its sole discretion.
  • You will not upload, post or otherwise transmit any Entry or content that contains software viruses, Trojan horses, worms, time bombs, cancelbots, or any other computer code or files that are designed to disrupt, damage, or limit the functioning of any software or hardware.
  • You are not submitting any confidential, proprietary, or trade secret information.

 

“Intellectual Property Rights” means any and all tangible and intangible: (i) copyrights and other rights associated with works of authorship throughout the world, including without limitation copyrights, neighboring rights, moral rights, and mask works, and all derivative works thereof; (ii) trademark and trade name rights and similar rights; (iii) trade secret rights; (iv) patents, designs, algorithms, utility models, and other industrial property rights, and all improvements thereto; (v) all other intellectual and industrial property rights (of every kind and nature throughout the world and however designated) whether arising by operation of law, contract, license, or otherwise; and (vi) all registrations, applications, renewals, extensions, continuations, divisions, or reissues thereof now or hereafter in force (including any rights in any of the foregoing) ; and (vii) any licenses, permissions and consents associated with (i) through (vi) above.

 

2. SPONSOR AND ADMINISTRATOR: Sponsor and Administrator: Stryker Corporation, 325 Corporate Drive, Mahwah, New Jersey 07430, and Ketchum, Inc., 1285 Avenue of the Americas, New York, New York 10019.

 

3. ELIGIBILITY: To be eligible to enter or win the Contest, each Entrant must be 21 years of age or older, and not a Healthcare Professional (defined as someone who provides healthcare services to patients and can purchase, lease, recommend, use, or arrange for the purchase or lease of medical devices). If the Entrant is submitting an Entry for the Entrant to win the Prize, then the Entrant must be a legal resident of the State of Florida in one of the below Designated Areas of Florida at the time of submitting an Entry. If the Entrant is submitting an Entry for the Entrant’s loved one to win the Prize, then the Entrant’s loved one must be a legal resident of the State of Florida in one of the below Designated Areas of Florida at the time of submitting an Entry.  Void where prohibited, taxed, or restricted by law.  Employees of Stryker, Ketchum, Inc. (“Ketchum”), and of each of their respective agencies, parents companies, subsidiaries, affiliates, promotional partners, sales representatives, and promotion and other vendor agencies involved in this Contest (collectively “Released Parties”) and the immediate family (defined for the purposes of this Contest as parents, spouses, siblings, and children) and household members of all such employees, are NOT eligible to participate in the Contest and/or win a Prize. The “Designated Areas” of Florida only includes residents of Duval County, Florida.  For any Entry containing a description of why the Entrant’s loved one needs a push to go to the doctor, the loved one described in the Entry must also meet all criteria, including eligibility criteria, herein.  All Entrants must be able and willing to schedule a visit to the doctor on their own, without any assistance of the Released Parties, and at a time that is mutually convenient for the winning Entrant and Prize described below.  Through this Contest or these Official Rules, Stryker is not making any representation about the doctor that each Entrant chooses, in no way endorses the chosen doctor in any regard, and does not have a preference for the Entrant’s decision of his/her doctor.  Any doctor chosen is in the Entrant’s sole discretion and is not considered to be affiliated with Stryker in any way.  The Contest is subject to all applicable federal, state, and local laws and regulations.  To the extent the individual who needs a push to go to the doctor (the Entrant or Entrant’s love one) must pay a fee to visit the doctor, that individual is solely responsible for paying such a fee.  Participation in this Contest constitutes Entrant’s full and unconditional agreement to these Official Rules, which are final and binding in all matters related to the Contest.  Sponsor’s decisions are final and binding in all matters relating to this Contest, including without limitation to interpretation and application of these Official Rules. 

 

4. WINNER SELECTION:  A panel of judges at Ketchum that has the required knowledge and experience to apply the selection criteria will select one winner based upon the following criteria: 40% for following the theme of the Contest, 30% for originality, and 30% for exhibiting Stryker’s brand attributes: responsive, energetic, and determined. Stryker will determine the accuracy of information provided in the winning Entry and will inform Ketchum if an Entrant is disqualified for any reason.  In the event that there is a tie, the judging panel will determine the winner based upon the above criteria and will break the tie with a vote among its judging panel.  In the event of a dispute among individuals claiming each is the legitimate entrant, an Entry will be deemed made by the natural person who is the holder of the email account associated with the Entry.

 

5. PRIZE:  The winning Entry will receive the following: at a mutually convenient location and time in Duval County on July 17, 2018, Stryker and Fred Funk will pick up the individual who needs a push to go to the doctor (the Entrant or the Entrant’s loved one) and drive that individual and a guest (in total, two (2) people) to the doctor for a doctor’s visit for the one individual who needs a push to go to the doctor, scheduled by the Entrant or Entrant’s loved one (“Prize”). The total approximate value of the Prize is:  $500.  All taxes and tax reporting requirements and consequences associated with use and acceptance of the Prize (if any) are the sole responsibility of the winner. 

 

6. WINNER NOTIFICATION: Winner will be notified by June 25, 2018, and in no event later than within 30 days after the end of the Contest Period by email and/or phone. Inability to contact a winner after 7 days or a winner rejecting his/her Prize may result in the Prize being awarded to an alternate winner, time permitting. 

 

7. GENERAL CONDITIONS:

 

a. Contest Entrants hereby grant Stryker a non-exclusive, perpetual, worldwide license to use, broadcast, publish, store, reproduce, distribute, syndicate, modify, create derivative works from, and otherwise use and exhibit the Entry in all media now known and later come into being for any purpose whatsoever without further compensation and without any further notice than provided herein. This includes but is not limited to use of entries online including Stryker’s website, on Stryker’s social media channels including Facebook and Instagram, via email distribution, and via distribution to media outlets.

 

Contest Entrants hereby also grant Stryker the ability to use the right of publicity for Entrant and/or Entrants’ loved one, including but not limited to name, likeness, photograph, opinions, hometown, and/or state without further payment or consideration for promotional purposes in any media worldwide. 

 

Entrants represent and warrant that they have full legal right, power, and authority to grant Stryker the foregoing licenses and rights of publicity and if applicable, have secured all necessary rights from any participants in, and contributors to, their Entry in order to grant such a license, including without limitation from Entrants’ loved ones.

 

b. Stryker is under no obligation to use any Entry or return the Entries to Entrants.

 

c. The winner and winner’s guest will be required to execute and return an Affidavit of Eligibility, Consent, and General Release form, appropriate tax documents (if required), and authorization for a criminal background check within 7 calendar days of delivery of the documents. Non-compliance within this time period may result in disqualification and selection of an alternate winner, time permitting. Further proof or verification of identity and eligibility may be required along with a records check. Stryker reserves the right to disqualify the potential Prize winner in its sole discretion in accordance with these Official Rules and also based on the results of the criminal background check.

 

8. USE OF CONTEST INFORMATION: Stryker shall be able to use any and all Contest Entries and information in accordance with the licenses granted above. Stryker reserves the right to use any and all information related to the Contest, including Entries and rights of publicity provided by the Entrants, for editorial, marketing, and any other purpose, unless prohibited by law.  

 

9. NOT ENDORSED BY FACEBOOK OR INSTAGRAM: By participating in this Contest, you acknowledge that this Contest is in no way sponsored, endorsed or administered by, or associated with, Facebook or Instagram and release Facebook and Instagram from any and all liability arising from or related to this Contest. The information you are providing for this Contest is being provided to Stryker and not to Facebook or Instagram, and will be used to notify you if you have won.

 

10. CONDUCT: All Contest participants agree to be bound by these Official Rules and the decisions of the judges.  Stryker in its sole discretion reserves the right to disqualify any person it finds to be tampering with the Entry process, the operation of its website, or is otherwise in violation of these Official Rules.

 

11. LIMITATIONS OF LIABILITY: STRYKER IS NOT RESPONSIBLE FOR ANY HUMAN, COMPUTER, ONLINE, TELEPHONE, TECHNICAL, OR MECHANICAL MALFUNCTIONS THAT MAY OCCUR. IF FOR ANY REASON THE CONTEST OR PRIZE IS NOT CAPABLE OF RUNNING AS PLANNED, INCLUDING DISRUPTIONS CAUSED BY COMPUTER VIRUS, BUGS, TAMPERING, UNAUTHORIZED INTERVENTION, FRAUD, TECHNICAL FAILURES, FIRE, FLOOD, EPIDEMIC, EARTHQUAKE, EXPLOSION, LABOR DISPUTE OR STRIKE, ACT OF GOD OR PUBLIC ENEMY, SATELLITE OR EQUIPMENT FAILURE, RIOT OR CIVIL DISTURBANCE, WAR (DECLARED OR UNDECLARED), TERRORIST THREAT OR ACTIVITY, OR ANY FEDERAL, STATE OR LOCAL GOVERNMENT LAW, ORDER OR REGULATION, ORDER OF ANY COURT OR JURISDICTION, OR ANY OTHER CAUSE THAT CORRUPTS, AFFECTS OR COMPROMISES THE ADMINISTRATION, SECURITY, FAIRNESS, INTEGRITY, PROPER CONDUCT OR INTENDED PARTICIPATION OF THE CONTEST (EACH A “FORCE MAJEURE” EVENT), SPONSOR RESERVES THE RIGHT AT ITS SOLE DISCRETION TO DISQUALIFY ANY INDIVIDUAL SPONSOR DEEMS RESPONSIBLE AND SPONSOR FURTHER RESERVES THE RIGHT TO CANCEL, TERMINATE, MODIFY, OR SUSPEND THE CONTEST OR PRIZE (OR PORTION THEREOF). THIS ALSO INCLUDES WITHOUT LIMITATION THE UNAVAILABILITY OF FRED FUNK OR AN INSUFFICIENT NUMBER OF ENTRIES TO MEET THE MINIMUM SELECTION CRITERIA. DISPUTES: Entrants agree that any and all disputes, claims, and causes of action arising out of or connected with this Contest or the Prize, other than those concerning the administration of the Contest or the determination of the winner, shall be resolved individually, without resort to any form of class action. Any disputes shall be resolved exclusively by and in the state or federal courts located in Essex County, New Jersey without regard to conflicts of laws provisions. Any and all claims, judgments, and/or awards will be limited to actual third-party, out-of-pocket costs incurred (if any) not to exceed $250, but in no event will attorneys’ fees be awarded or recoverable.  Entrants expressly and knowingly waive all right to seek, and under no circumstances will any Entrant be allowed to obtain an award for, punitive, incidental, consequential or special damages, lost profits and/or any other damages, other than actual out-of-pocket expenses not to exceed $250, and/or any rights to have damages multiplied or otherwise increased.  Remedies for entrants are limited to a claim for money damages (if any) and the Entrant waives any right to seek injunctive or equitable relief.

Stryker will determine the accuracy of information provided in the winning Entry and will inform Ketchum if an Entrant is disqualified for any reason.  In the event that there is a tie, the judging panel will determine the winner based upon the above criteria and will break the tie with a vote among its judging panel.  In the event of a dispute among individuals claiming each is the legitimate entrant, an Entry will be deemed made by the natural person who is the holder of the email account associated with the Entry.

 

5. PRIZE:  The winning Entry will receive the following: at a mutually convenient location and time within the Designated Areas of Pennsylvania on June 12, 2018, Stryker and Jerome Bettis will pick up the individual who needs a push to go to the doctor (the Entrant or the Entrant’s loved one) and drive that individual and a guest (in total, two (2) people) to the doctor for a doctor’s visit for the one individual who needs a push to go to the doctor, scheduled by the Entrant or Entrant’s loved one (“Prize”). The total approximate value of the Prize is:  $1,500.  All taxes and tax reporting requirements and consequences associated with use and acceptance of the Prize (if any) are the sole responsibility of the winner. 

 

6. WINNER NOTIFICATION: Winner will be notified by May 28, 2018, and in no event later than within 30 days after the end of the Contest Period by email and/or phone. Inability to contact a winner after 7 days or a winner rejecting his/her Prize may result in the Prize being awarded to an alternate winner, time permitting. 

 

7. GENERAL CONDITIONS:

 

a. Contest Entrants hereby grant Stryker a non-exclusive, perpetual, worldwide license to use, broadcast, publish, store, reproduce, distribute, syndicate, modify, create derivative works from, and otherwise use and exhibit the Entry in all media now known and later come into being for any purpose whatsoever without further compensation and without any further notice than provided herein. This includes but is not limited to use of entries online including Stryker’s website, on Stryker’s social media channels including Facebook and Instagram, via email distribution, and via distribution to media outlets.

 

Contest Entrants hereby also grant Stryker the ability to use the right of publicity for Entrant and/or Entrants’ loved one, including but not limited to name, likeness, photograph, opinions, hometown, and/or state without further payment or consideration for promotional purposes in any media worldwide. 

 

Entrants represent and warrant that they have full legal right, power, and authority to grant Stryker the foregoing licenses and rights of publicity and if applicable, have secured all necessary rights from any participants in, and contributors to, their Entry in order to grant such a license, including without limitation from Entrants’ loved ones.

 

b. Stryker is under no obligation to use any Entry or return the Entries to Entrants.

 

c. The winner and winner’s guest will be required to execute and return an Affidavit of Eligibility, Consent, and General Release form, appropriate tax documents (if required), and authorization for a criminal background check within 7 calendar days of delivery of the documents. Non-compliance within this time period may result in disqualification and selection of an alternate winner, time permitting. Further proof or verification of identity and eligibility may be required along with a records check. Stryker reserves the right to disqualify the potential Prize winner in its sole discretion in accordance with these Official Rules and also based on the results of the criminal background check.

 

8. USE OF CONTEST INFORMATION: Stryker shall be able to use any and all Contest Entries and information in accordance with the licenses granted above. “2018 Stryker Get on the Bus New Jersey Contest” Official Rules (“Official Rules”)

1. HOW TO ENTER:  To enter the “2018 Stryker Get on the Bus New Jersey Contest” (“Contest”) visit patients.stryker.com (“Site”) during the Contest Period, accurately and truthfully complete the online registration form, accept these Contest Rules, and follow the instructions for submitting an Entry as described on the Site and below. An Entry must be an original essay of 200 words or less in the English language describing why the Entrant or Entrant’s loved one needs an extra push to visit the doctor. The Entry must be submitted through the Site.  The party accepting these Contest Rules and submitting an Entry is referred to as “Entrant” or “you.”  Proof of submitting an Entry other than as detailed in these Official Rules (such as a printed or copied confirmation screen or message) does not constitute actual receipt of the Entry for purposes of this Contest.

 

The Contest will begin April 10, 2018 at 12:01 AM Eastern Daylight Time (“EDT”) and conclude July 20, 2018 at 11:59 PM EDT (“Contest Period”).  All Entries must be submitted within the Contest Period.  The winning Entry will be selected by July 30, 2018.  Stryker Corporation (“Stryker” or “Sponsor”) is the official keeper of time for this Contest. 

 

Each Entrant represents and warrants as follows:

 

  • Each Entry does not infringe or otherwise violate any rights of any third party or entity, including without limitation Intellectual Property Rights as defined below, defamation, privacy, publicity, false light, misappropriation, confidentiality, or any contractual or other rights.
  • Your entire Entry is an original work by you, and you have all the rights, licenses, permissions, and consents necessary to submit the Entry and to grant all of the rights that you have granted hereunder.
  • Your Entry does not contain content that is inconsistent with the permissible uses outlined by these Contest Rules, including without limitation content that is unlawful, harmful, threatening, abusive, harassing, defamatory, libelous, invasive of another’s privacy, vulgar, profane, sexually explicit, obscene, racially or ethnically offensive, or otherwise objectionable as determined by the Sponsor in its sole discretion.
  • You will not upload, post or otherwise transmit any Entry or content that contains software viruses, Trojan horses, worms, time bombs, cancelbots, or any other computer code or files that are designed to disrupt, damage, or limit the functioning of any software or hardware.
  • You are not submitting any confidential, proprietary, or trade secret information.

3. ELIGIBILITY: To be eligible to enter or win the Contest, each Entrant must be 21 years of age or older, and not a Healthcare Professional (defined as someone who provides healthcare services to patients and can purchase, lease, recommend, use, or arrange for the purchase or lease of medical devices). If the Entrant is submitting an Entry for the Entrant to win the Prize, then the Entrant must be a legal resident of the State of New Jersey in one of the below Designated Areas of New Jersey at the time of submitting an Entry. If the Entrant is submitting an Entry for the Entrant’s loved one to win the Prize, then the Entrant’s loved one must be a legal resident of the State of New Jersey in one of the below Designated Areas of New Jersey at the time of submitting an Entry.  Void where prohibited, taxed, or restricted by law.  Employees of Stryker, Ketchum, Inc. (“Ketchum”), and of each of their respective agencies, parents companies, subsidiaries, affiliates, promotional partners, sales representatives, and promotion and other vendor agencies involved in this Contest (collectively “Released Parties”) and the immediate family (defined for the purposes of this Contest as parents, spouses, siblings, and children) and household members of all such employees, are NOT eligible to participate in the Contest and/or win a Prize. The “Designated Areas” of New Jersey only includes residents of the New Jersey counties of Bergen County, Essex County, Hudson County, or Passaic County.  For any Entry containing a description of why the Entrant’s loved one needs a push to go to the doctor, the loved one described in the Entry must also meet all criteria, including eligibility criteria, herein.  All Entrants must be able and willing to schedule a visit to the doctor on their own, without any assistance of the Released Parties, and at a time that is mutually convenient for the winning Entrant and Prize described below.  Through this Contest or these Official Rules, Stryker is not making any representation about the doctor that each Entrant chooses, in no way endorses the chosen doctor in any regard, and does not have a preference for the Entrant’s decision of his/her doctor.  Any doctor chosen is in the Entrant’s sole discretion and is not considered to be affiliated with Stryker in any way.  The Contest is subject to all applicable federal, state, and local laws and regulations.  To the extent the individual who needs a push to go to the doctor (the Entrant or Entrant’s love one) must pay a fee to visit the doctor, that individual is solely responsible for paying such a fee.  Participation in this Contest constitutes Entrant’s full and unconditional agreement to these Official Rules, which are final and binding in all matters related to the Contest.  Sponsor’s decisions are final and binding in all matters relating to this Contest, including without limitation to interpretation and application of these Official Rules. 

 

4. WINNER SELECTION:  A panel of judges at Ketchum that has the required knowledge and experience to apply the selection criteria will select one winner based upon the following criteria: 40% for following the theme of the Contest, 30% for originality, and 30% for exhibiting Stryker’s brand attributes: responsive, energetic, and determined. Stryker will determine the accuracy of information provided in the winning Entry and will inform Ketchum if an Entrant is disqualified for any reason.  In the event that there is a tie, the judging panel will determine the winner based upon the above criteria and will break the tie with a vote among its judging panel.  In the event of a dispute among individuals claiming each is the legitimate entrant, an Entry will be deemed made by the natural person who is the holder of the email account associated with the Entry.

The total approximate value of the Prize is:  $1,400.  All taxes and tax reporting requirements and consequences associated with use and acceptance of the Prize (if any) are the sole responsibility of the winner.

IMPORTANT INFORMATION ABOUT HIP AND KNEE REPLACEMENT

Hip joint replacement is intended for use in individuals with joint disease resulting from degenerative and rheumatoid arthritis, avascular necrosis, fracture of the neck of the femur or functional deformity of the hip.

Knee joint replacement is intended for use in individuals with joint disease resulting from degenerative, rheumatoid and post-traumatic arthritis, and for moderate deformity of the knee. Joint replacement surgery is not appropriate for patients with certain types of infections, any mental or neuromuscular disorder which would create an unacceptable risk of prosthesis instability, prosthesis fixation failure or complications in postoperative care, compromised bone stock, skeletal immaturity, severe instability of the joint, or excessive body weight.

Indications: The Scandinavian Total Ankle Replacement (STAR Ankle) is indicated for use as a non-cemented implant to replace a painful arthritic ankle joint due to osteoarthritis, post-traumatic arthritis or rheumatoid arthritis.

Contra-Indications: Active or prior deep infection in the ankle joint or adjacent bones, Skeletal immaturity, Bone stock inadequate to support the device including: Severe osteoporotic or osteopenia condition or other conditions resulting in poor bone quality , Avascular necrosis of the talus, Prior surgery and / or injury that has adversely affected ankle bone quality, Malalignment or severe deformity of involved or adjacent anatomic structures including: Hindfoot or forefoot malalignment precluding plantigrade foot, Significant malalignment of the knee joint. Insufficient ligament support that cannot be repaired with soft tissue stabilization, Neuromuscular disease resulting in lack of normal muscle function about the affected ankle, Lower extremity vascular insufficiency demonstrated by Doppler arterial pressure, Charcot joint or peripheral neuropathy that may lead to Charcot joint of the affected ankle, Prior arthrodesis at the ankle joint , Poor skin and soft tissue quality about the surgical site.

Common Side Effects from Total Ankle Replacement Surgery: For the first two weeks after surgery it is normal to have a moderate amount of pain. You may need to use pain medicine(s). This pain may slowly decrease over time, but it is not unusual to experience some discomfort for up to three months and swelling may continue for up to a year after surgery. Contact your surgeon right away if at any time you notice: Fluid leaking from your wound, Redness around your wound, Pain or swelling that starts suddenly (especially after an ankle twist or fall) or Severe pain after the initial two weeks following your surgery.

The T2 Ankle Arthrodesis Nail is intended for tibiotalocalcaneal arthrodesis (fusion) and to provide stabilization of the hindfoot and ankle including the transverse tarsal joints coupling the mid-foot to the hindfoot. Examples of specific indications include: Post-traumatic or primary arthrosis, previously infected arthrosis (second degree), revision of Failed Ankle arthrodesis, failed Total Ankle Replacement, avascular Necrosis of the Talus (requiring tibiocalcaneal arthrodesis), Neuroarthropathy or Neuromuscular Deformity or other neuromuscular disease with severe deformity or instability of the ankle, rheumatoid arthritis with severe deformity such as rheumatoid hindfoot, osteoarthritis, nonunions or pseudarthrosis of hindfoot and distal tibia, malunited tibial pilon fracture, charcot foot, severe endstage degenerative arthritis, severe defects after tumor resection pantalar arthrodesis.

Contraindications: The physician’s education, training and professional judgement must be relied upon to choose the most appropriate device and treatment. Conditions presenting an increased risk of failure include: any active or suspected latent infection or marked local inflammation in or about the affected area. Compromised vascularity that would inhibit adequate blood supply to the fracture or the operative site. Bone stock compromised by disease, infection or prior implantation that can not provide adequate support and/or fixation of the devices. Material sensitivity, documented or suspected. Obesity. An overweight or obese patient can produce loads on the implant that can lead to failure of the fixation of the device or to failure of the device itself. Patients having inadequate tissue coverage over the operative site. Implant utilization that would interfere with anatomical structures or physiological performance. Any mental or neuromuscular disorder which would create an unacceptable risk of fixation failure or complications in postoperative care. Other medical or surgical conditions which would preclude the potential benefit of surgery. The T2 Ankle Arthrodesis Nail should NOT be used if following conditions are present: tibial malalignment of > 10˚ in any plane, severe vascular deficiency, osteomyelitis or soft tissue infection.

Indications: The Stryker VariAx 2 Wrist Fusion System is indicated for wrist arthrodesis and fixation of fractures in patients with wrist arthritis or fractures of other small bones of the carpus. Specific indications include:

ReUnion System Indications for Use:

For use as a Hemi or Total Shoulder Replacement: Aseptic necrosis of the humeral head; Painful, disabling joint disease of the shoulder resulting from: degenerative arthritis, rheumatoid arthritis or posttraumatic arthritis; Proximal humeral fracture and/or dislocation; Clinical management problems where arthrodesis or alternative reconstructive techniques are less likely to achieve satisfactory results; Revision of previous unsuccessful total shoulder replacement, resurfacing or other procedure.

Glenoid components are intended for cemented use only. The humeral stem components are intended for both cemented and cementless use. In the case of revision, when ReUnion TSA humeral stems are well fixed, the system is indicated for conversion to a total shoulder arthroplasty. In conjunction with ReUnion TSA humeral and glenoid components, if the natural glenoid provides sufficient bone stock, ReUnion TSA humeral stems can be converted from a hemiarthroplasty to a total shoulder arthroplasty, as well as revised from an existing total shoulder arthroplasty to a secondary total shoulder arthroplasty. It is also indicated for conversion to a hemiarthroplasty. In conjunction with ReUnion TSA humeral components, ReUnion TSA humeral stems can be converted from a total or ReUnion RSA reverse shoulder arthroplasty to a hemiarthroplasty, as well as revised from an existing hemiarthroplasty to a secondary hemiarthroplasty, in treatment of previously failed shoulder arthroplasty cases where revision to a reverse shoulder arthroplasty is inappropriate.

ReUnion Reverse Arthroplasty System (RSA): The ReUnion RSA Shoulder System is intended for primary, fracture, or revision of total Shoulder replacement. The patient’s joint must be anatomically and structurally suited to receive the selected implant(s), and a functional deltoid muscle is necessary to use the device. The patient’s joint must have gross rotator cuff deficiency, a functional deltoid muscle and be anatomically and structurally suited to receive the selected implant(s). Painful, disabling joint disease of the shoulder resulting from: degenerative arthritis or rheumatoid arthritis; Proximal humeral fracture; Revision of previously failed shoulder joint replacement; Glenoid Baseplate components are intended for cementless use with the addition of screw fixation; The Humeral Stem components are intended for both cemented and cementless use; In the case of revision, when ReUnion TSA humeral stems are well fixed, the system is indicated for conversion to a reverse shoulder arthroplasty.

In conjunction with ReUnion RSA humeral and glenoid components, ReUnion TSA humeral stems can be converted from a hemi or total shoulder arthroplasty to a reverse shoulder arthroplasty, as well as revised from an existing reverse shoulder arthroplasty to a secondary reverse shoulder arthroplasty, in treatment of a grossly deficient rotator cuff with sever arthropathy or previously failed joint replacement with a grossly deficient rotator cuff. The patient must have a functional deltoid muscle, and be anatomically and structurally suited to receive the implant(s).
